Medical:

BlueCrossBlueShield of Georgia. You can choose from 2 different plans, a base or a buy-up. The plans co-pays and deductibles vary. Prescription coverage is included on both.

Dental:

Metlife. Preventative services are covered at 100%. Basic services are covered at 80% after deductible. Major services are covered at 50% after deductible.

Vision:

Vision Service Plan (VSP). Benefits include: routine vision exams with $10 co-pay, lenses every 12 months with $25 co-pay, $120 allowance for frames every 24 months, and $120 allowance for contacts every 12 months.

Flexible Spending Account (medical & dependent care):

Ceridian. Employees can save approximately 25% of each dollar spent on medical and/or dependent care expenses by participating in the pre-tax deductions with the FSA.

Cancer Insurance:

Allstate. This plan is supplemental insurance designed to help off-set non-medical expenses associated with the treatment of cancer and other specified diseases. It pays in addition to any other coverage and is paid directly to the insured.

Accident Insurance:

UNUM. This plan has enrollment in August each year. It pays a lump sum benefit based on the type of injury (or covered incident) you sustain or the type of treatment you need.
